Woodlands Primary Healthcare is a growing primary care practice seeking a detail-oriented, self-motivated Medical Assistant to support our Chronic Care Management (CCM) and Principal Care Management (PCM) programs remotely. This is an excellent opportunity for a bilingual MA who thrives in a patient-centered environment and is comfortable working independently from home.
Conduct monthly CCM/PCM outreach calls with enrolled patients to review symptoms, medications, and care plan progress.
Accurately document all encounters and time logs in eClinicalWorks (ECW).
Coordinate care between specialists, PCP, and ancillary providers.
Assist with care plan creation and updates under provider supervision.
Identify and escalate urgent patient concerns to the clinical team.
Maintain compliance with CMS billing requirements for CCM/PCM time tracking.
Support patient enrollment and consent documentation.
Medical Assistant certification (CMA, RMA, or equivalent) preferred.
Minimum 2–3 years of experience in a clinical or care coordination role.
Proficiency in eClinicalWorks (ECW) — required, no exceptions.
Experience with CCM, PCM, or RPM programs strongly preferred.
Bilingual — English/Spanish required (spoken and written).
Must reside in the Spring, TX or Katy, TX area.
Reliable home office setup with stable internet connection.
Strong communication, documentation, and time‑management skills.
Ability to work independently with minimal supervision.
Pay: $17–$25/hour depending on experience.
Full‑time or part‑time options available.
